New Patients

Primary Care services are available, and we are accepting New Patients at this time. We recommend scheduling a visit that is not a prevention visit (or annual visit) because the visits don't allow us to meet your needs at times, and don't cover prescription refills, problems to discuss, or help us get to know you.

Family Medicine for All Ages

Family Medicine is a specialty that gets to take care of all ages and the whole family. This includes newborn and pediatrics through your last days. We also have an on-site lab with Quest, and are supported by our own clinical team with a Clinical Pharmacist as well as a Health Coach for nutrition, stress, and more. 

Costs, Forms, & Patient Portal

Every healthcare clinic has forms and a patient portal. The main forms are for New Patients, but you may need to request medical records, send us a consent to take care of your child, or learn what the policies are for some medications. Costs of care are also available and the patient portal has many features to help you.

Community Partnerships

We work with private practices, specialists, hospitals, nonprofits, community-based organizations, and universities to better take care of our patients. These organizations help us by providing services, conducting practice-based research on topics that matter to us and you, and also keep us independent, privately-owned. Once a portion of this website is ready, we will share these groups with you all. We also hope to start a Patient Advisory Council to continuously improve the patient experience.

Currently, Westminster Medical Clinic, a smaller, private practice in Westminster, Colorado, supports our practice in both clinical and administrative ways.

7AM - 6PM
Visit with her for help in nutrition, meal planning, stress, mood, and more. She is in person monthly, but available by televisit throughout the month. Call or send a message in the patient portal to schedule. She works with Dr. VanWyk and Alyssa in your care. They are all in the loop.

2nd Fri every month

Monica, Clinical Pharmacist
8AM - 5PM
Visit with her and improve high blood pressure, high sugar in your blood, or review mediations and side effects to help with. She is also available to see you for Annual Wellness Visits (Medicare), and works with Dr. VanWyk and Alyssa in your care. They are all in the loop.
